PRHS Clinic Procedures

When a student is not feeling well and/or sick, a student must report directly to the Student Relations Office to see the nurse/clinic. The nurse will evaluate a student and inform parents/guardian of the student’s condition. The nurse or administrator will contact parents/guardians should a student need to be picked up from the school.  

  • Once a student has permission from their teacher to see the nurse, a student will check-in to the Student Relations office. Should a student feel sick during lunchtime, the student must report directly to Student Relations.  
  • A student must go to the Student Relations Office to see the nurse.
  • A student will use the keypad to check-in into the clinic as soon as the student is done seeing the nurse. The student will check-out using the keypad in the nurse’s office.  
  • A student is NOT allowed to leave the school campus without check-in and being seen by the nurse.  
  • Should a student reach out to their parent/guardian please inform your student to directly report to Student Relation to see the nurse.  
  • Please ensure that all contact information is current and updated consistently.  
  • Log in to CCPS Portal to double check your current contact information and emergency contacts too.  
  • Parent/guardian please ensure your phone numbers and emergency contact are updated. Current contact information is how we get hold of a parent/guardian in a timely manner.  

Medications: Daily and Emergency

If a student requires daily or emergency medication during the school day, a District Medication Authorization Form must be completed and signed by the health care provider each school year.  

  • The parent/guardian must bring all medications to the school with the completed medication authorization form.  
  • Medications must be in the original container and labels must match the health care provider’s order.

Nursing Procedures at School

Please inform the school nurse if your child requires any special nursing procedures such as diabetes care, tube feeding, catheterization, etc. It is important to have the appropriate form completed by your child’s healthcare provider and bring it to school with any supplies that are needed for the procedure.
